Indonesia AKL Registration for Pen Injectors: Who Holds the Licence and What Breaks First

Published 2026-10-28 · Keywords: Indonesia AKL registration

In Indonesia no foreign company can hold your device registration. The AKL is issued to an Indonesian legal entity — your licence holder — and that entity, not your device supplier, carries the obligation to the Ministry of Health. This single structural fact explains most of the delays Western teams hit in the market: they treat the AKL as a document the manufacturer produces, when in fact it is a licence the manufacturer supplies evidence for and someone else holds.

Read the file backwards and it gets easier. Ask what the holder needs from the device side, in the order the ministry asks for it, and the program becomes a document exercise with a known critical path.

Three parties, three different jobs

Party What they hold What they can lose
Manufacturer (your device supplier) ISO 13485 system, design and test documentation, declaration of conformity, letter of authorization Nothing at ministry level — but every document supplied is filed under the holder’s name
Licence holder (importir, with a PJT) The AKL itself, the registered warehouse, the technical responsible person The licence — and with it your entire market access
Distributor Commercial rights, import execution, payment Nothing regulatory, unless they are also the holder

The PJT — penanggung jawab teknis, the technical responsible person — is the detail that separates a serious holder from an opportunistic one. A PJT qualified for pharmaceuticals is not automatically accepted for devices, and a holder without a device-qualified PJT cannot file properly.

What the AKL actually is

An AKL (izin edar alat kesehatan) is a distribution licence for a device, issued by the Ministry of Health and numbered in the form KEMENKES RI AKL … for imported devices — AKD for devices manufactured in Indonesia. It is valid for up to five years and renewable.

Three things are fixed to that number: the holder, the manufacturing site, and the product name as written on the application. Change any one of them and you are not amending a file — you are filing again. This is why the distributor-change question in year three matters more than the price negotiation in year one.

Risk class sets the depth, not the route

Indonesia classifies devices A to D by risk, and every class goes through the same AKL route — the class changes how much evidence is reviewed, not which door you walk through.

Class Typical profile What changes in the file
A Low risk, non-invasive Administrative completeness dominates
B Low-to-medium risk Technical documentation reviewed in substance
C Medium-to-high risk Fuller technical file, tighter scrutiny of test evidence
D High risk Highest evidence burden, longest review

The ministry makes the classification decision, not the applicant. If you believe the class is wrong, that is a conversation with the reviewer backed by a written rationale — not an argument, and never a reason to submit under a class you know is wrong and correct it later.

For a pen injector, the class follows the device’s own risk profile and how it is presented. Where the device is part of a drug-device combination, the drug side is a separate filing with its own requirements — the AKL covers the device.

The device-side documents, and where supplier packs stall

This is the part a device supplier controls, and it is where most first submissions lose a month. Not because documents are missing, but because they disagree with each other.

Document Issued by The failure mode we see
ISO 13485 certificate with scope text Certification body Scope says “components” or excludes design — reads as a different business than the one being registered
Certificate of Free Sale (CFS) Competent authority in the country of manufacture Product named differently from the AKL application
Letter of authorization Manufacturer Names the holder’s previous company name, or says “all products” instead of the product family
IFU and label in Bahasa Indonesia You, with the manufacturer Translation finished after printing plates were cut
Sterilization validation summary Manufacturer Summary exists but cannot be traced to the batch and load pattern
Test reports (ISO 11608 parts, ISO 10993-1) Manufacturer or test house Reports reference a platform variant that is not the one being registered
Shelf-life and stability summary Manufacturer Real-time and accelerated data mixed without saying which is which

The three delays that are not the ministry’s fault

Name mismatch. The CFS, the letter of authorization and the application each carry a product name. When they differ by a hyphen or a model suffix, the file goes back. Fix the naming convention before anyone notarizes anything.

Late label translation. Bahasa Indonesia labeling is mandatory, and the translation has to be reviewed as content — dose units, storage statements, warnings — not merely produced. If the plates are already cut, a correction costs a reprint cycle, not a PDF edit.

A holder who is not operationally ready. The holder needs the registered warehouse and the PJT in place at submission, not at approval. Programs that appoint a holder on a letter of intent and sort the warehouse out afterwards discover the sequencing problem at the worst moment.

A dated sequence you can plan against

Stage Owner Planning range
Holder appointment, letter of authorization, legalization You 3–6 weeks
Device document set assembly Manufacturer 2–4 weeks if the pack already exists; 8+ if reports must be reissued
Submission and administrative review Holder Weeks — the ministry publishes service times, but queue position is not something a supplier can promise
Corrections round Holder + manufacturer Budget one; it is the stage nobody plans for
AKL issued, then import approval per shipment Holder After approval, every shipment is its own step

These are planning ranges from programs we have supplied device documentation for, not service guarantees. The authority sets its own clock, and no supplier — ours included — can shorten it. What a supplier can do is make sure the file is not the reason the clock restarts.

Five questions to ask before you sign with a holder

  1. Does your PJT cover medical devices, or only pharmaceuticals?
  2. Which warehouse is registered, and is it the one my goods will actually be stored in?
  3. If we change distributor in year three, does the AKL move with us or stay with you?
  4. Who pays the registration and renewal fees, and what happens if they lapse?
  5. What exact product name will appear on the AKL?

Question three is the one worth an argument. A holder who will not commit in writing to cooperating with a transfer is telling you what year three looks like.

What the device supplier should hand you

For an Indonesian file we provide: the ISO 13485 certificate with the scope text quoted in full — certified for finished pen injectors, design through distribution, not components; MDSAP audit evidence covering five authorities; per-part ISO 11608 test reports; ISO 10993-1 biocompatibility; a container compatibility summary naming your cartridge or pre-filled syringe supplier; and a letter of authorization in the form your holder needs.

Certificate copies travel with the order documentation. Before that point you get the substance in writing: which standards apply to the platform you are evaluating, who holds each certificate, the certificate number, the issuing body and the validity date.

What this does not cover

Drug registration, if your product is a combination; the holder’s own company licensing; customs and import execution; and post-market vigilance reporting, which sits with the holder and is the obligation most often forgotten after launch. The device-side document set is on the Southeast Asia market page; the scope wording question is covered in reading an ISO 13485 scope.

Next step: send your intended risk class, the product name exactly as it will appear on the label, and your holder’s legal entity name. We return the device document list mapped to those three inputs within one working day. Start the inquiry.
